Have your say in our Live Well client feedback survey 2023-2024

Live Well is a free 12-week programme which is designed to help eligible Croydon residents improve their health and wellbeing.

Our advisers provide face to face, telephone or video call support to help residents quit smoking, lose weight and/or reduce their alcohol consumption.

Live Well works with residents to:

  • devise personalised programmes
  • help them make meaningful and sustainable lifestyle changes
  • provide face to face, telephone or video call support to boost their chances of success

You recently completed the Live Well 12-week programme - we would now like to invite you to have your say and complete our short survey. Your views and feedback are really important and help us to find out how we can make any improvements, so that we can deliver a high quality and effective service.

The results will be shared with the Live Well team, commissioners and the senior management team. This will help us make improved decisions about our service.

We do not need to collect your contact information for this survey and will not be asking for this. When taking part, please keep your responses about the Live Well programme only. We will analyse the responses and report findings from this anonymous survey without any of your personal details to make improvements/changes to our services.
