You may have already heard on the news, on social media, via friends and family that the COVID-19 vaccine is now widely available to young people aged between 12-15 years and also 16-17 years. Vaccines are a key tool in the fight against COVID-19 and getting back to doing the things we love, safely.

The public health team at Croydon Council want to explore your views and opinions on the COVID-19 vaccines. We want you to have your say and invite you to complete a short survey so we understand what is important to you. The information you provide us with, will help us to offer the right information on vaccines and develop services that meet the needs of the young people in Croydon.

All the information you provide will remain confidential and anonymous.

We really value your input and everyone filling in this survey will be entered into a prize draw. The winner will be able to choose from one of the following prizes:

  • Parkour course for one person
  • 1-hour DJ taster session or a recording studio experience

Please provide your mobile number or phone number at the end of the survey so we can contact you. This information will not be used for any other purpose.
