Welcome to our short survey!

The Croydon 17 - 25 year olds Care Experienced Young People and Adults nurse service works with care leavers to:

• devise personalised programmes
• help care leavers make meaningful and sustainable lifestyle changes
• provide face to face, telephone or video call support

As you have recently had contact with our Croydon 17 - 25 year olds Care Experienced Young People and Adults nurse service and/or had a health assessment, we would now like to invite you to have your say and take part in our short survey which should take no more than 5 – 10 minutes to complete. Please let us know how you feel now that you have left care or are about to leave. Your views and feedback are really important and helps us to find out how we can deliver a high quality and effective service for young people who are leaving care.

Results from the survey will be shared with the senior management team and commissioners. We will not share your personal details with them but there is a separate section in the survey which will allow you to tell us anything you may be worried about. You will need to confirm this.
