Welcome to our short survey! We hope you enjoy completing it.

We would like to find out how you feel about your life and where you are living at the moment, since you left foster care recently.

This form will be saved to your file that we have on record in our Children Looked After (CLA) services . If there is anyone you don’t want it to be shared with, please let us know. Sometimes CLA managers and care experienced older and younger people might use information from these surveys to learn about how we are doing in general and how we have been providing a good service to looked-after children. This would always be done anonymously, so your data would still be confidential (no-one can find out who you are unless you say so).

This survey will take place from 9 April 2024 to 30 September 2024 - thank you for taking part. If you complete this survey, you can choose to be entered into a Prize Draw for £30 Amazon vouchers. One person will be randomly selected from the four age groups that the survey is being carried out with. Don’t forget to fill in your consent at the end of the survey if you wish to be entered into the Prize Draw. By agreeing to be entered into the Prize Draw, please provide your mobile number or phone number at the end of the survey so we can contact you. This information will not be used for any other purpose.
